• SMART on FHIR for Device Integration
• RedOx for EHR Integrations
• Receive patient information from EHR via Orders resource
• Keep the patient aligned in the Program (like Screening, Management or Reassessment)
• Assign and activate validated BP Device (like Bluetooth or Cellular) to the patient
• Process the BP readings received from the Patient
• Receive anti-Hypertension medication information of the Patient from the EHR using the Clinical Summary resource
• Send the report at the end of the assigned program date to the EHR via Results resource
• Built Patient Engagement capabilities where the Health Coach:
  • Set Patient Communication Availability preferences (like Weekday/Weekends and Morning/Afternoon/Evening)
  • Set Patient Communication Preference (like Home/Work/Cell Phone for calls and Sensly/Cell Phone to receive Messages or Nudges)
  • Create Notes to manage the Patients information
  • Create Activities to manage the activities performed with the Patient like Phone call, Messages etc.
  • Set Patient Motivation levels depending on how motivated the patient is to participate in the program in providing the BP readings as per requirement
  • Send Nudges to keep the patient well participated in providing the readings and successful in completion of the Program
